Position Details

GEM Program

Social Worker – Grade 2

Permanent Part-Time, 8 hours per week

 

An exciting opportunity exists to provide allied health and support services to the West Gippsland Healthcare Group Geriatric Evaluation and Management (GEM) Program which will be based at Neerim South Hospital.  The Allied Health staff will be supported by the WGHG Allied Health Departments and Continuing and Complex Care Program or other programs as required and work as part of a multidisciplinary team to provide services to patients on the GEM Program.
