An exciting opportunity is available for a Social Worker to provide services at the West Gippsland Hospital. The ideal candidate will have a degree in Social Work and be eligible for a membership with Australian Association of Social Workers. The primary focus of the position will be to ensure a team approach to the psychosocial assessment, planning and implementation of the discharge care needs of the patient in the acute setting.
