Position Details

A new and exciting opportunity exists for a suitably qualified and skilled Registered Nurse to provide a liaison role between admitted and community palliative care.  This is a temporary (12 months) full time position. 

This Grade 4 role will coordinate services between admitted and non-admitted services to ensure that patient care is delivered in a streamlined, multidisciplinary and timely manner, as well as being responsible for education, implementation and monitoring of the ‘end of life' pathway. 

The successful applicant will have a post-graduate qualification in Palliative Care/Oncology or a minimum of five years post registration experience including three years minimum experience in Palliative Care with the willingness to undertake the relevant post-graduate qualification. Demonstrated high level skills as a clinician and resource person in palliative care, a commitment to ongoing self-development and the ability to work independently as well as within a team is also required for this position. 

The completion or preparedness to complete Advance Care Planning is also essential.
