About the role

South West Healthcare's (SWH) Regional Palliative Care Team provides best practice support services clients (including outpatients, acute and subacute inpatients) and their families, to assist patients and their families cope with the effects of terminal illness, grief, loss and associated trauma.

As an experienced Social Worker / Counsellor / Palliative Care Professional, this role will work collaboratively in a multi-disciplinary team to undertake a range of assessments and contribute to the clinical planning of individual care for each client and their family. You will support the development and maintenance of a high quality care environment and promote and participate in quality improvement activities and initiatives.

We have opportunities for full-time and part-time roles, following various internal progression and expansion of the team. 

What you bring

As an experienced Palliative Care practitioner with a minimum of 5 years' experience and ideally holding post graduate qualifications (or working towards), you will have demonstrated experience and skills in delivering a broad range of effective and efficient counselling services to individuals and their families, including comprehensive psychosocial assessments. Working as part of a collaborative and highly skilled team of professionals, excellent verbal and written communications skills are essential, and a comprehensive understanding and knowledge of legislation, regulation (including Palliative Care Australia and Bereavement Standards) and contemporary practice is a must.
