Are you an Allied Health Professional or Registered Nurse looking for something different? 

An opportunity exists for Allied Health Professionals or Registered Nurses for part time or full time applications for a position in the Discharge Support & Liaison team. Applications are encouraged from Occupational Therapists, Physiotherapists, Social Workers, Speech Pathologists, Dieticians and Registered Nurses with an interest in helping patients access appropriate services to enable timely discharge from hospital. The Discharge Support & Liaison team is responsible for working collaboratively within ward based multidisciplinary clinical teams to facilitate patient flow from the admitted hospital setting to the most appropriate subacute pathway to prevent readmission and ensure that all patients receive the right care at the right time and in the right place

If you have an interest in helping clients access support in the community this may be the role for you. 

Whilst this position is full time, part time applications may be considered
