About the role

South West Healthcare's (SWH) Community Palliative Care Department provides community based palliative care and consultancy services throughout the Barwon South West sub-region.

We are seeking a motivated Palliative Care Nurse Practitioner to join our passionate and skilled team. This new role will strengthen the current service model along with working alongside the Palliative Care Physician to provide high level specialised clinical care, support specialist and generalist palliative care clinicians; providing advanced clinical support and leadership through evidence-based practice and professional development.  The role will utilise a holistic care approach that is about treating the whole patient and family to support the development and maintaining of a high quality care environment and promote and participate in quality improvement activities and initiatives.

Our position is part-time ongoing and is 56 hours per fortnight.

Working as part of a collaborative and highly skilled team of professionals, excellent verbal and written communications skills are essential, and a comprehensive understanding and knowledge of legislation, regulation and contemporary practice is a must.

Employment Terms and Conditions will be in line with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2016-2020, with the appropriate classification Nurse Practitioner Grade 5.
