About the role

South West Healthcare's (SWH) Community Palliative Care Department provides community based palliative care throughout the Barwon South West sub-region.

We are seeking a motivated Palliative Care Nurse Practitioner to join our team, this role will provide a high level of specialised clinical care and coordinate case management, clinical problem solving and decision making. You will support the development and maintaining of a high quality care environment and promote and participate in quality improvement activities and initiatives.

The position is part-time, 56 hours per fortnight.

Working as part of a collaborative and highly skilled team of professionals, excellent verbal and written communications skills are essential, and a comprehensive understanding and knowledge of legislation, regulation and contemporary practice is a must.

Employment Terms and Conditions will be in line with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2016-2020, with the appropriate classification Nurse Practitioner Grade 5.

The successful applicant will be required to consent and undergo a Police Record's and Working with Children's Check and provide evidence of current Immunisation status in alignment with South West Healthcare's Immunisation Policy.
