Clinical Nurse Consultant – Heathcote Health

Fixed Term Part Time (0.7 FTE) – 12 month contract, flexible hours of work based in Heathcote and Bendigo

About Heathcote Health

Heathcote Health is an integrated rural health service, providing a broad range of acute, aged care, primary health programs and home-based services to Heathcote and surrounding districts.

This Role

Heathcote Health is looking to appoint a Clinical Nurse Consultant (CNC) for a fixed term part-time role within a Nurse Practitioner-led Disability model of care. This exciting position forms a key part of a project that aims to develop, implement, and evaluate a Nurse-Practitioner-led outreach disability model of care across the Greater Bendigo area that will include the development of COVID-19 care pathways. Primary goals for this model of care are to improve the coordination of services, increase access to primary health care assessments and screening, early identification of issues, prevention of functional decline and exacerbation of existing physical and psychosocial issues for people with a disability.

This role would suit an experienced registered nurse, with excellent patient assessment and communication skills, with the ability to think critically about complex clinical scenarios. Opportunity exists for upskilling into a supported Nurse Practitioner Candidacy if the applicant is interested.  Current nurse practitioners and medical officers at Heathcote Health will provide clinical support and mentoring.

Whilst undertaking the project, Heathcote Health will explore ongoing funding opportunities to offer ongoing employment/candidacy.
