About the role

South West Healthcare's (SWH) HARP team are seeking suitably qualified, experienced Registered Nurses or Allied Health professionals with a minimum 5 years' experience in relevant acute/community setting for the position of Care Coordinator.

With a particular focus on people with chronic illness, The Hospital Admission Risk Program (HARP) aims to improve patient outcomes, reduce avoidable hospital admissions and ED presentations, and enable access to required services.

With some additional funding, SWH is trialling an extension of the reach of HARP across the region in partnership with Moyne Health Service. Working for SWH, the successful applicants will spend the majority of their time located at our neighbouring health service, working within their teams and communities to ensure equitable access to this valuable program.

The following roles are located at:

  • Moyne Health Service (0.4 FTE)
  • South West Healthcare (0.4 FTE)

These roles are being offered as 12 month fixed term contracts. The 0.4 roles can be combined to create 0.8 contracts.

What you bring

You will be currently registered with AHPRA with tertiary qualification/s in a related health discipline and a minimum 5 years' relevant experience. Applicants must have a high level of knowledge and skills in assessment, planning and implementation of services, and a demonstrated ability to work autonomously and within a team environment.

Effective communication skills, a working knowledge of community based support networks and services and advanced skills in time management, planning and priority setting, are also required for the position.
