About the role
South West Healthcare (SWH) is seeking a suitably qualified and experienced Registered Nurse with a minimum of five years' experience in relevant acute/community settings in the position of Care Coordinator.
The Care Coordinator assists to improve the care of people living in the community with chronic and complex illnesses by preventing avoidable hospital admissions and reducing the frequency and duration of hospital admissions. Care Coordinators collaborate with services in the community, acute, aged care and specialist care to reduce duplication and create effective health pathways for the client.
The following roles are located at:
- Moyne Health Service (0.4 FTE)
- Terang and Mortlake Health Service (0.4 FTE)
- Timboon and District Healthcare Service (0.4 FTE)
The positions are fixed term part time until June 2026.
What you bring
- You will be currently registered with AHPRA with tertiary qualification/s in a related health discipline and a minimum 5 years' relevant experience.
- Applicants must have a high level of knowledge and skills in assessment, planning and implementation of services, and a demonstrated ability to work autonomously and within a team environment.
- Effective communication skills, a working knowledge of community-based support networks and services and advanced skills in time management, planning and priority setting, are also required for the position.

Employment Terms and Conditions will be in line with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020-2024 with the appropriate classification of Community Health Nurse.
Salary Range: $102,278.80 per annum pro rata.
