About the role

South West Healthcare (SWH) is seeking suitably qualified and experienced Registered Nurses or Allied Health Professionals with a minimum of three years' experience in relevant acute/community settings to apply to the position of Care Coordinator within our Emergency Department.

The Care Coordinator facilitates improved patient flow within the Emergency Department and Emergency Observation Unit through the provision of holistic assessment and comprehensive discharge planning. The role is responsible for ensuring that upon discharge patients have appropriate community supports and referrals in place to enable them to manage safely in their environment and receive access to follow up treatment.

  • Fulltime (80 hours per fortnight) with a monthly ADO

  • Salary Range: $97,650.80 - $111,566.00 pro rata , plus superannuation

What you bring

You will be currently registered with AHPRA with tertiary qualification/s in the related health discipline and a minimum 3 years' relevant experience. Applicants must have a high level of knowledge and skills in assessment, planning and implementation of services, and a demonstrated ability to work autonomously and within a team environment.

Effective communication skills, a working knowledge of community based support networks and services and advanced skills in time management organisation, planning and priority setting are also required for the position.
