Position Details

About the role

The Rehabilitation in the Home (RITH) program is a newly implemented program at South West Healthcare (SWH) recognising the benefits of enabling patients to rehabilitate in their home environment.

 

Rehabilitation care, is care in which the primary clinical purpose or treatment goal is improvement in the functioning of a patient with impairment, activity limitation or participation restriction due to a health condition. Rehabilitation care can be provided in either the inpatient setting or the home.

 

Reporting to the Nurse Unit Manager, District Nursing Services, the Rehabilitation in the Home nurse will work as a member of the multidisciplinary RITH team, using contemporary standards and evidence-based practice to support nursing and a multidisciplinary approach of the program. The role incorporates the following but is not limited to, unsupervised clinical practice, patient advocacy and co-ordination of individual patient care requirements within the RITH team.

  • Salary Range: $102,278.80 per annum, plus superannuation

 

What you bring

The successful applicant will be a Registered Nurse holding a current Registration Certificate as required by Australian Health Practitioner Regulation Agency and have relevant acute experience of at least five (5) years as a Registered Nurse. Some subacute experience preferred but not essential

 

 You will have a demonstrated commitment to professional confidentiality and remaining consumer focused at all times.

 

You will provide guidance and leadership to all levels of staff in the delivery of quality care within a safe, therapeutic environment, knowledge of wound management and a demonstrated understanding of person-centred care.
