Position Details

About the role

  • Provide service coordination and case management for patients with inflammatory bowel disease, hepatitis, and decompensated liver disease.
  • Follow up with patient's post-endoscopy.
  • Offer support, treatment options, and education for patients with GI health conditions and related clinical alcohol and drug issues.
  • Develop networks with General Practitioners and allied health providers in the South West region.
  • Engage in a shared care model with specialist clinics and facilitate early primary care management.
  • Deliver specialist outreach services to remote areas via telehealth.
  • Liaise with health and welfare agencies, including mental health, alcohol and other drugs, and Aboriginal services.

Position overview

  • Full time (80 hours per week), On-going position
  • Part time applicants will be considered

  • Registered Nurse Grade 2, minimum of 5 years clinical experience

  • Serve as a clinical nursing/midwifery resource and advisor to ward staff and South West healthcare service providers.
  • Participate in team planning and development activities within the clinical specialty area.
  • Undertake projects and research to address specified clinical nursing needs.
  • Adhere to professional standards, codes, and behaviours as legislated by the Nursing Midwifery Board of Australia (NMBA), other relevant professional bodies, and SWH Policy, Standards, and Practice Guidelines.
