Position Title: Community Health Nurse – Living Well
Classification: ZJ1
Salary per hour: $55.97 + superannuation + generous salary packaging options
Employment Type: Ongoing part-time 48 hours per fortnight
Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028
Role description
Are you passionate about delivering community-based care and promoting health equity? Dhelkaya Health is seeking a dedicated Community Health Nurse to join our Living Well team in the Mount Alexander Shire.
With a focus on older adults, this is a dynamic role where every day is a chance to make a positive impact on the community.
About you
In this role you will:
- Develop and implement health promotion initiatives aligned with the Victorian Public Health and Wellbeing Plan.
- Engage with diverse community groups to deliver culturally safe and inclusive care.
- Collaborate with multidisciplinary teams to address social determinants of health.
- Provide clinical services, including assessments, care planning, and chronic disease management.
- Hold a current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
