About the Role

Are you passionate about empowering vulnerable families and making a real difference in their lives? South West Healthcare is seeking a compassionate and motivated Healthy Mothers, Healthy Babies (HMHB) Outreach Worker to join our Social Work and Counselling team.

In this rewarding role, you will be supporting pregnant women facing psychosocial challenges to enhance health and wellbeing outcomes for both them and their babies. Many of the women you will work with have complex health and social needs that require tailored support and coordinated services.

The HMHB program aims to reduce health inequities by addressing maternal risk factors and providing vital support throughout pregnancy. As a valued member of our multidisciplinary team, your responsibilities will include:

  • Enhancing access to antenatal, postnatal, and related health and human services
  • Conducting psychosocial and risk assessments, care coordination, and referrals
  • Facilitating or linking women to group-based peer support activities
  • Providing support throughout pregnancy with a focus on holistic care
  • Promoting healthy behaviours through key health education messages
  • Delivering assertive outreach across a variety of settings, including homes, clinics, and hospitals
  • Educating women on the impacts of smoking, alcohol, drug use, nutrition, and breastfeeding

 Position Overview

  • Permanent part time, 80 hours per fortnight (Job Share Applications considered)
  • Warrnambool Campus
  • Classification:  Allied Health Professional Grade 2 / Community Health Nurse (based on qualification)
  • Salary Range: $88,467 - $107,390 pro rata, plus superannuation

What You Bring

We are looking for someone who is genuinely committed to supporting vulnerable families and improving outcomes for women and children. You will have:

  • Tertiary qualifications in Social Work, Midwifery, or Community Nursing
  • Experience in community-based outreach and engaging with vulnerable families
  • A background in working with women, infants, and children
  • Strong interpersonal and communication skills to connect with clients and stakeholders
  • The ability to work autonomously and collaboratively within a multidisciplinary team
  • A reflective, compassionate approach and willingness to participate in clinical supervision
  • A current Victorian Driver's Licence
