Position: Cognitive Dementia and Memory Service (CDAMS) Clinician

  • Employment Type: Part-time (24 hours per fortnight) fixed term until 18 January 2027
  • Location: Warrnambool Campus
  • Classification: Registered Nurse, Social Worker, Occupational Therapist, Psychologist or Speech Pathologist Grade 2 (or equivalent)
  • Salary Range: $84,058.00 - $104,379.60 pro rata, plus superannuation

About the role

South West Healthcare (SWH) is seeking a motivated and experienced clinician to join our Cognitive Dementia and Memory Service (CDAMS) team. CDAMS provides specialist, multidisciplinary assessment, diagnosis, treatment, support, referral, and information for people experiencing undiagnosed cognitive difficulties and dementia.

Our service runs monthly at the Warrnambool Campus, bringing together a team of Medical Physicians (e.g. Geriatricians) and allied health and Neuropsychologists under the guidance of the CDAMS Clinical Coordinator.

CDAMS is a sub-acute ambulatory care service, funded by the Victorian Department of Health and Human Services, and operates under a statewide model, ensuring evidence-based, patient-centred care. Joining CDAMS offers clinicians the opportunity to work within a highly supportive, collaborative, and expert multidisciplinary team, making a real difference in the lives of people with cognitive and memory challenges.

We are looking for an experienced Nurse, Occupational Therapist, Psychologist, Social Worker, or Speech Pathologist who:

  • Has a passion for working in a multidisciplinary setting to support people with cognitive difficulties and dementia.
  • Demonstrates strong interpersonal, communication, and time management skills.
  • Is committed to providing high-quality, person-centred care in a collaborative team environment.
  • Can contribute to assessment, diagnosis, treatment planning, support, and referral services.
