Role description
Dhelkaya Health's at Home Programs deliver high-quality clinical care to patients in their own homes or places of residence, providing safe alternatives to hospital-based care. The Residential in Reach (RIR) program operates across public and private aged care facilities, working closely with RACFs, GPs, and health teams to ensure timely and effective care for older persons.
We are seeking an experienced and passionate RIR Lead to provide operational and clinical leadership within the RIR program. This role is pivotal in coordinating care, managing clinical operations, and supporting aged care facilities to deliver evidence-based, person-centred care.
Key Responsibilities
- Lead and manage day-to-day operations of the RIR program, including workload allocation and coordination of RACF visits.
- Provide clinical leadership and specialist assessment for residents, including triage advice and acute illness management.
- Collaborate with RACFs, GPs, and multidisciplinary teams to develop and implement individualised care plans.
- Support early intervention strategies to prevent avoidable hospital presentations.
- Act as a key liaison for RACF staff and advocate for optimal health outcomes for residents.
- Promote health education and ensure coordinated care across all phases, including transition and discharge planning.
About you
- Registered Nurse with current AHPRA registration.
- Extensive clinical experience in aged care, acute care, or community health settings.
- Strong leadership and team coordination skills.
- Ability to work collaboratively with multiple stakeholders and deliver high-quality, evidence-based care.
- Excellent communication and problem-solving skills.
