Are you passionate about helping older people regain confidence, independence, and quality of life after a hospital stay? We're looking for a motivated Allied Health Assistant (AHA) to join our multidisciplinary Transition Care Program (TCP) team.
About the Role
As an AHA, you'll work under the guidance of our Allied Health Professionals to deliver low-intensity therapeutic activities in both hospital and home settings. You'll support clients to improve their physical, cognitive, and social functioning—always aligned with their individual goals and care plans. The role also includes assisting with general administration, equipment management, and daily operational tasks.

About TCP
TCP provides short-term, goal-focused care that helps older people transition safely from hospital while improving or maintaining function. At CHRH, we deliver this program on behalf of Grampians Health Services and the Victorian Department of Health.

Details of Appointment: Part-Time, fixed-term position working 40 hours per fortnight from date of commencement.
Salary: Dependent on qualifications and experience, as per award + superannuation
