Position Title: At Home Services Coordinator
Classification: Clinical Nurse Consultant A (ZF4)
Salary per hour: $59.79 + superannuation + generous salary packaging options
Employment Type: Ongoing part-time 48 hours per fortnight
Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028
Role description
Our At Home services provide an alternative to hospital-based care, enabling clients to receive clinical treatment and multidisciplinary support in their homes or place of residence. Current programs include:
- Hospital in the Home (HITH)
- Geriatric Evaluation Management (GEM) at Home
- Residential In Reach (RIR)
- Rehabilitation in the Home (RITH)
These programs operate 7 days a week, focusing on value‑based health care—improving client outcomes, enhancing the care experience, and supporting staff to deliver excellent care.
The At Home Nurse Coordinator is a pivotal Clinical Nurse Consultant role responsible for providing expert nursing leadership across all phases of care. You will:
- Coordinate screening, triage, and intake for At Home services
- Lead timely admission and initial assessment
- Develop, implement, and regularly review individualised care plans
- Support acute deterioration recognition and response
- Facilitate smooth transitions between At Home programs, inpatient units, residential care, and community services
- Partner with clients and carers to ensure care aligns with goals and preferences
- Promote At Home pathways and deliver staff education to build capability
- Develop collaborative relationships with internal and external stakeholders to support patient flow and ensure coordinated, high‑quality care
This role enables Dhelkaya Health to offer more care options, support flow across the health service, and ensure patients receive the right care, in the right place, at the right time.
About you
- Bachelor of Nursing with current AHPRA registration
- Minimum 3 years full time equivalent working as registered nurse in an acute setting or related setting
- Current Victorian drivers' licence
