About the role

South West Healthcare (SWH) is seeking a passionate and organised Coordinator - Social Support Group to lead engaging, person-centred programs for older people and people living with a disability in our community. Our program provides a safe, welcoming and inclusive environment where participants can build connections and take part in meaningful activities that promote independence, dignity and wellbeing.

The Coordinator oversees the day-to-day operations of the program, including supervising staff, liaising with participants and carers, coordinating volunteers, and ensuring services meet CHSP, HACC and NDIS funding and quality requirements. This is a rewarding opportunity to make a genuine difference while leading a vibrant, community-focused program.

What you bring

The successful applicant will demonstrate experience delivering group-based programs for older people and/or people with a disability, with a strong commitment to person-centred care that promotes independence, dignity and choice. You will bring the ability to develop meaningful activities and goal-directed care plans, alongside strong leadership, organisational and communication skills. Knowledge of NDIS, CHSP and HACC requirements, budget management and quality standards will be highly regarded.
