About the role

South West Healthcare (SWH) is seeking an experienced Diabetes Nurse Educator to join our Community Health Team within the Primary and Community Services Division.

This role supports clients across the full continuum of care, from inpatient settings through to community-based services. The Community Health Team delivers services across all SWH campuses, as well as contracted services outside the organisation.

As a Diabetes Nurse Educator, you will provide specialist clinical expertise in diabetes management, supporting patients, carers, and staff. You will act as a key resource for best-practice diabetes care and work collaboratively with multidisciplinary teams and community partners to ensure coordinated, high-quality care.

This is an excellent opportunity to make a meaningful impact on patient outcomes through education, clinical leadership, and integrated care delivery.

What you bring

  • A minimum of five years' post‑graduate nursing experience in a relevant specialty
  • A Graduate Certificate in Diabetes Education (or equivalent qualification)
  • Credentialed Diabetes Educator with the Australian Diabetes Educators Association (ADEA), or willingness to work towards credentialing
  • Demonstrated ability to work independently and as part of a multidisciplinary team
  • Strong communication, organisational, and computer skills, with a commitment to delivering high‑quality, person‑centred care
