HIP Health Coordinator (PAC and HARP)

Registered Nurse Grade 3B or Grade 2 Allied Health Professional

Part-time Permanent 48 hours per fortnight


Albury Wodonga Health – Wodonga Campus requires either a Registered Nurse or Allied Health Professional to join the Health Coordination Team within Health Independence Program (HIP). The role is to reduce presentations/admissions and optimise patient care through the provision of care coordination and integrated care planning.


HIP services are designed to reduce length of hospital based admissions by supporting the transition from hospital to home or to provide short term supports following an acute admission and to increase a person's self-management abilities to reduce unnecessary hospital admissions. A HIP Health Coordinator will support individual clients through the post acute continuum of health care as it relates to their specific chronic disease, multiple morbidities or complexity of needs.


Primary responsibilities of this role is to provide care coordination to achieve consistent care for clients between and across admitted and community based care, delivering care coordination that is short term to medium-long term.  Short term care coordination (from 4 weeks to 3 months) or medium-long term care coordination from 3 to 10 months for those clients with a greater complexity of needs.  


The successful clinician will be required to work flexibly across both Albury and Wodonga Campuses and possess initiative and drive to develop and negotiate new work and clinical practices within the health service and wider service system

Candidates meeting the following mandatory criteria are encouraged to apply:

  • Tertiary  qualifications or equivalent in Allied Health or Nursing discipline
  • Current registration with AHPRA or proof of eligibility for membership of relevant allied health professional group
  • Current Drivers licence
  • Substantial post graduate clinical experience

Salary and conditions of employment will be in accordance with either the Nurses and Midwives Enterprise Agreement 2014-2017 or Allied Health Professionals Enterprise Agreement 2014-2017.


Successful applicants will be required to complete appropriate Criminal Record Screening paperwork/processes in order for AWH to meet its obligations regarding screening for serious criminal offences (within and external to Australia).  Applicants unable to commit to this screening process should not submit an application for employment.


Successful applicants will be required to complete mandatory orientation modules using the AWH e-learning system prior to commencing employment.


This is a Category A position.  Successful applicants of Category A positions must provide evidence of vaccination prior to commencement.
