HIP Health Coordinator – Short Term Supports

 - Grade 3 Allied Health or Registered Nurse Grade 4A

Multiple Positions

12 month Maternity leave and

3 month LSL commencing November 2018


Albury Wodonga Health –Wodonga requires either an experienced Allied Health Professional or Registered Nurse to join our Health Coordination Team.  This position will work within a team that provides care coordination on a continuum by supporting individual clients through the post-acute continuum of health care as it relates to their specific disease, multiple morbidities or complex needs.


The HIP Health Coordinator supports clients to navigate the complex health and social care systems to overcome barriers to accessing quality care.  Care coordination – both short term and longer-term form part of the suite of services offered within the Health Independence Program.


Primary responsibilities of these locum role(s) will be to provide short-term care coordination and supports as co-ordinated brokered services for clients recently discharged from hospital, to aid in their recuperation and recovery.

Candidates meeting the following mandatory criteria are encouraged to apply:

  • Tertiary qualifications or equivalent in Allied Health and Nursing
  • Current registration with AHPRA or professional membership
  • Current Drivers licence
  • Substantial post graduate and clinical experience ( minimum 5 years)


Salary and conditions of employment will be in accordance with the Allied Health Professionals Enterprise Agreement (Vic) 2016-2020 and Nurses and Midwives Enterprise agreement (Vic)  2016- 2020.


Successful applicants will be required to complete appropriate Criminal Record Screening paperwork/processes in order for AWH to meet its obligations regarding screening for serious criminal offences (within and external to Australia).  Applicants unable to commit to this screening process should not submit an application for employment.


Successful applicants will be required to complete mandatory orientation modules using the AWH e-learning system prior to commencing employment.


This is a Category A position.  Successful applicants of Category A positions must provide evidence of vaccination prior to commencement.
