Position Details
- Help people with mental health needs plan life after prison.
- Guide safe and steady moves from custody into the community.
- Work with health and justice services to support real change.
- Gain strong experience in mental health and reintegration work.
- Fixed-term, full-time role for 12 months earning 81,496 - $98,955 per year, plus super.
What you will do
As assessment & transition coordinator – mental health, you will support people in custody with mental health needs. You will help them plan for release and connect to services.
You will:
- deliver targeted support to people with serious mental health needs
- build strong links with health, drug and alcohol, and community services
- create clear release plans that support safe return to the community
- complete accurate risk and needs assessments and support case reviews
- manage reliable data and share updates with the wider network.
About you
A tertiary qualification in a related field is desirable. Experience in human services or welfare work is highly regarded.
You should also have:
- a solid understanding of mental health and reintegration needs
- a trauma-informed approach and care for complex clients
- the ability to work well with Aboriginal communities and diverse groups
- strong computer, data and communication skills.
