We are looking for an experienced Clinical Nurse Consultant to provide expert clinical leadership and consultancy in complex care. If you have a passion for improving the outcomes of clients with complex and/or chronic disease, this is a great chance to expand your skills and knowledge.   

Contract: Ongoing, 80.0 hours per fortnight

The position:

In this role you will facilitate service access, undertake comprehensive client assessment and coordinate the development and implementation of the multi-disciplinary goal-based care plan. This position will ensure best practice in care coordination, service delivery and efficiency of resource usage to ensure delivery of multi-disciplinary client-centred care that promotes health independence and prevents unnecessary presentations or admission to hospital.

Key skills you need:

  • Current APHRA registration as a Registered Nurse with a minimum of 5 years acute nursing experience
  • Recent nursing experience in chronic or complex disease management with advanced clinical assessment, screening and referral ability
  • A high level of experience in client and carer education within the chronic disease management framework.
  • Good understanding of the community services available to support health independence
