Position Details

Short Term contract 13/12/2021 – 30/01/2022

Part Time, 48.0 hours per fortnight

An exciting opportunity exists for a skilled and experienced Registered Nurse to perform this Grade 4 role for a fixed term.  If you have a passion for improving the outcomes of patients with complex and/or chronic disease, this is a great opportunity to expand your skills and knowledge.   

The Position:

In this role you will facilitate service access, undertake comprehensive client assessment and coordinate the development and implementation of the multi-disciplinary goal-based care plan.  This position will ensure best practice in care coordination, service delivery and efficiency of resource usage to ensure delivery of multi-disciplinary patient-centred care that promotes health independence and prevents unnecessary presentations or admission to hospital. 

The Department:

The Complex Care Program sits within the Community Nursing Department and provides specialised, client-centred clinical care and care coordination in the community/ambulatory setting through an integrated response of hospital and community services.  Complex Care is governed by the Health Independence Program (HIP) guidelines.

Key skills you need:

  • Current APHRA registration as a Registered Nurse with a minimum of 5 years acute nursing experience
  • Recent nursing experience in chronic or complex disease management with advanced clinical assessment, screening and referral ability
  • Sound understanding of patient self-management and proven ability to provide strategies for symptom understanding and management
  • A high level of experience in patient and carer education within the chronic disease management framework.
  • Good understanding of the community services available to support health independence
