Position Details

STATUS:            Ongoing, Part Time positions available

HOURS:             1.1 EFT available to split into 2 roles

GRADE:              Grade 2/Level 1

DEPARTMENT:   Home Based Nursing Services

Two permanent vacancies have arisen within our Home Based Nursing Services department for skilled and experienced Registered Nurses to join the team in a Level One role.  These roles provide an exciting opportunity for talented nurses to grow and consolidate their skills and knowledge in community nursing.

In this role you will deliver quality nursing care to a broad range of clients across a range of funding streams including Commonwealth Home Support Program (CHSP) nursing services, wound care, and palliative care.  You will perform comprehensive assessments, care planning and care coordination and providing appropriate BRHS and Community services referrals.  Applicants for this position must be available to undertake weekend, afternoon shifts and on call duties as required.

Required criteria includes:

  • Completion of Graduate Nursing year plus a minimum of 3 years' nursing experience
  • Ability to undertake patient assessments and develop goal-based, person-centred care plans.
  • Demonstrated competency in clinical skills and be up to date in mandatory competencies.
  • Knowledge in key community nursing care practices including wound care and diabetes management.
  • Demonstrated ability to communicate effectively with a wide range of individuals, including co-workers, clients/patients and other health professionals.
  • Proven ability to work effectively, both individually and within a team, and proven competency in the appropriate escalation of care.
  • Knowledge of handling chemotherapy disconnections and appropriate management of chemo spills and hazardous chemical protocols.
  • Competent and proven skills in accessing and de-accessing PICCs and Ports
  • Commitment to commence post graduate studies in areas relevant to Home Based Nursing activity such as Palliative Care, Wound Care or High Acuity Care within two years of appointment.
