A skilled and motivated Registered Nurse is sought to join our Home Based Nursing (HBN) team in a Grade 3 position.  This role offers a great opportunity for a talented nurse to progress their career at a senior level.

The Contract: Fixed Term until 3/09/2023, Part Time 64.0 hours per fortnight.  A split of these hours will be considered.

The Service:

The Home Based Nursing Service extends the provision of nursing care into the community through Hospital in the Home, Post-Acute Care, Palliative Care, Residential in Reach (RIR), and community based nursing services funded through the Home and Community Care Program for Younger People (HACC-PYP) and the Commonwealth Home Support Program (CHSP).

The Position:

In this role you will deliver quality nursing care and care coordination to a broad range of clients ensuring comprehensive assessment, care planning and implementation in a competent and compassionate manner.  As a Level 2 nurse you will provide expertise to the HBN team, have the capacity to provide leave coverage for the RIR service as well as leadership to the HBN team by acting in charge as required.   Applicants for this position must be available to work weekend and afternoon shifts and on call duties as required. 

What you need:

  • A minimum of 3 years' acute nursing experience and at least 12 months experience in District Nursing (or comparable community or emergency nursing experience).
  • An ability to undertake advanced patient assessments and develop a goal-based, person-centred care plan.
  • An advanced level of knowledge in key community nursing care practices including wound care and diabetes management.
  • An excellent understanding of the principles and clinical requirements relating to palliative care.
  • Ability to communicate effectively with a wide range of individuals, including co-workers, clients/patients and other health professionals.
  • Experience handling chemotherapy disconnections and the appropriate management of chemo spills and hazardous chemical protocols.
  • Proven skills in accessing and de-accessing PICCs and Ports and IV cannulation.
