STATUS: Part Time, Fixed Term
HOURS: 48.0 hours per fortnight
An exciting opportunity has arisen to broaden your nursing career gaining experience in this autonomous and rewarding care coordination role.
The Position:
As an experienced nurse you will work closely with the site Hospital In The Home (HITH) Team Leader to support holistic operational planning and delivery of effective patient care within the COVID Pathways system. You will use your knowledge and expertise to coordinate and deliver quality communication and care to patients in their home setting using principles of evidence-based practice to improve patient outcomes and service delivery. HITH provides patient care in the home with patients classified as admitted and the program enabling them to receive high quality medical, nursing and allied health care in their own place of residence. This position is to be worked across a 7 day a week rolling roster.
What you need:
- Current AHPRA registration as a Registered Nurse
- Minimum 3 years' acute nursing experience
- Proven ability to undertake thorough patient assessments and develop goal-based, person-centred care plans
- Demonstrated ability to communicate effectively and compassionately with a wide range of individuals, including co-workers, clients/patients and other health professionals.
- Proven ability to work effectively, both individually and within a team, and proven competency in the appropriate escalation of care.
- Willingness to learn the COVID Pathways / COVID Monitor System and referral processes.
