About the role:
This role is key to supporting the delivery of SCV's sentinel event and review program. This program identifies emerging systemic issues in health services, clinical safety risks and generates actionable insights to improve care quality and safety across the Victorian health system.
Reporting to the Manager, Sentinel Event and Safety Reviews, the Senior Sentinel Event and Safety Review Officer assists health services with identifying, reporting and managing sentinel events and facilitates safety reviews using contemporary, evidence-informed methodologies that are both timely and thorough.
The Senior Sentinel Event and Safety Review Officer synthesises insights from multiple sources such as incident and clinical outcome data, workforce, safety culture and consumer experience system signals, and organisational behaviour, to identify clinical, cultural, organisational, or process-related contributors to harm in healthcare.

This administrative office works closely with divisions across the department. It engages with clinicians and consumers to help health services deliver better and safer healthcare.
About you:
Can you demonstrate the following?

Support the planning, coordination, and execution of high-quality, no-blame sentinel event and safety reviews in a high performing multidisciplinary team
-
Applying evidenced based and contemporary analysis methodologies and utilising expertise in human factors, systems thinking, and safety science to identify contributory factors and promote system-level learning and harm reduction

Collaborate closely with multidisciplinary colleagues across SCV and the health system to identify emerging trends, safety risks, and systemic issues, and support the development and implementation of proactive and targeted safety interventions

Contribute to the development, refinement, and continuous improvement of SCV's sentinel event and safety unit and to the review of policies and legislation ensuring alignment with national standards, evidence-based practices, and principles of psychological safety and just culture

Prepare high quality, accurate and timely reports, briefs, and presentations that communicate complex findings in a clear and accessible manner to support decision-making by the executive, the minister and internal and external stakeholders
Qualifications / Specialist Expertise
- A tertiary qualification in a relevant field such as Healthcare, Safety Science, Human Factors, Clinical Governance, Psychology, Systems Engineering, health policy reform or a related discipline is essential

Postgraduate qualifications or additional training in human factors, safety science, systems thinking, quality improvement, health system policy or clinical risk management are highly desirable
- Demonstrated application of quality improvement methodologies is highly desirable

Previous experience working in the health sector including understanding complex healthcare systems and clinical contexts and the ability to engage effectively with clinicians and health service executive is desirable

Demonstrated knowledge of or experience facilitating serious clinical incident or health system reviews utilising validated and evidence-based methodologies such as Root Cause Analysis, London Protocol, or similar system-focused approaches

Demonstrated knowledge of or experience applying human factors, systems thinking, and safety science principles to sentinel event investigations and/or health system reviews

Demonstrated ability to analyse and interpret health data, trends and performance metrics is desirable
- Demonstrated experience in participating in health system policy development, legislative review or project management would be highly regarded
