An exciting opportunity exists to join a dynamic team as an Advanced Case Manager with the Disability Client Services Team in the Western Melbourne Area. As an Advanced Case Manager you will be working with individuals, families and other stakeholders to achieve positive outcomes, and make a difference to the lives of some of Victoria's most vulnerable.
