Position Summary:

The Department of Health and Human Services is facilitation a place-based response to strengthen youth and community engagement in Melton, Brimbank and Wyndham.

Working in partnership with the community, the department is establishing a Community Support Groups (CSG) to provide a coordinated approach to program delivery to enhance collaboration, referrals and service delivery across a suite of programs to better support South Sudanese your people and their families.

This position, based Brimbank Melton Area will provide outreach support to West Melbourne Area, will play a key role in working with two auspice agencies that have been selected to manage the SCG and the programs and services to be delivered. The position will also be responsible for working with government agencies, local councils and other organisation at the local area level to support and complement the work of the CSGs and will assist the auspice agencies to address priority issues.

The position will work closely with the Office for Youth (OFY), the central coordination unit for CSGs across the state to ensure that overarching strategies, procedures and practices are delivered at the local area level in terms of the operation of the CSGs Meeting with and reporting on progress to OFY will be required on a regular basis.
