Position Summary:

Reporting to the Health Director in the West Division, this position plays a key leadership role. The role is accountable for the day to day management of the Health Integration & Partnerships Barwon-South Western unit. The role focuses on leading the delivery of agreed work plans, in collaboration with stakeholders and staff, continuous service monitoring, quality improvement, improving models of service delivery and a focus on innovative approaches to health, including population health outcomes, in collaboration with funded agencies and the broader health sector.

The role actively works in collaboration with the relevant central office program areas and other internal stakeholders in influencing, developing and implementing departmental policy, ensuring that local needs are taken into account in policy and program development.

** The operational division is currently in the process of formally consulting on an organisational change proposal that may see the responsibilities of this role change to include a broader scope of responsibility across health and human services.  The information provided below describes the situation as it currently is.
