Position Summary: 

As part of the Population Health and Community Wellbeing Team West Division this position contributes to the DHHS strategic directions of earlier and more connected support, and local solutions. The position aims to improve the wellbeing of individuals and communities through the development of opportunities for social and economic participation and to create opportunities for individuals and families to build better lives and achieve their potential by participating in the community and economy.

The Population Health and Community Wellbeing team works at a population level in prevention, health promotion, population health and wellbeing and service coordination and integration. The team supports prevention and population health and wellbeing to be reflected in the work of health and human services teams across the area and division. The role is involved in a range of key projects and initiatives that require community, inter-sectoral and whole-of-government engagement, collaboration and action to address the broad range of determinants that shape health and wellbeing. This includes enacting place-based approaches which consider the unique characteristics and interactions of people and place when designing and implementing interventions and or investment in each location or community. The position is an excellent opportunity for a person with conceptual and analytic ability, stakeholder and change management skills to implement a strong population health and wellbeing approach to contribute to outcomes for the local area.
