Position Details

HIP Care Coordinator

  • Permanent Part Time (0.5EFT)
  • A Health Independence Care Coordinator works in both Complex Care and Post Acute Care developing key relationships with patients and their carers through the streams of care
  • Coordinating short and long term individually tailored packages of health care and community services to people who need additional support for recuperation following discharge from hospital
  • A great opportunity to develop chronic illness management skills in a supportive environment with peer support and case conferencing
  • Nursing and Allied Health Practitioners are welcome to apply

Applications are sought from suitably qualified Registered Nurses (Div. 1), Occupational Therapists, Social Workers, Dieticians and Physiotherapists Year 2 and above, with previous experience in Chronic Disease Management to work in the IPAC Department at Echuca Regional Health. 

Key responsibilities include:

  • Provision of care coordination: negotiating, purchasing and monitoring the provision of community based supports by contracted providers.
  • Health coaching
  • Rapid response to manage multiple referrals internal and external within short time frames
  • Ensuring people recovering from a medical illness or with chronic and complex health needs receive appropriate services to maximise their health
  • IT skills to capture reportable data requirements 

Our team requires someone who has:

  • Qualifications in Nursing (Div 1), Occupational Therapy, Social Work, Dietetics or Physiotherapy
  • Proven experience as a care coordinator desirable
  • High-level awareness of HIP guidelines and referral pathways
  • Demonstrated knowledge  and passion to help make a difference in health care 

This position requires completion of a satisfactory National Police Check prior to appointment.
