Position Details
About the Role
Ongoing full time role with the provision of monthly accrued days off.
The Support at Home Manager will work with the Community Health Manager to undersatnd, prepare and transition the organisation's Home Care Packages, Short Term Restorative Care and Commonwealth Home Support services to the new Support at Home program. This will be achieved through the following:
- An in-depth review of the Support at Home manuals and guidance documents
- A detailed review of Moyne Health Services business processes.
- Development and implementation of a business impact statement and change program, along with guidance and recommendations to the Executive and Senior Managers.

About You
Minimum qualifications:
- Nursing, Allied Health or Community Services qualification
Minimum Level of experience required:
- Minimum 5 years experience in health related discipline
Key Selection Criteria:
- Previous experience managing teams, client assessment, care planning and case management
- Knowledge of My Aged Care, Support at Home program, NDIS and the Single Assessment Service
- Experience delivering Aged Care services in community settings
- Knowledge of the Strengthened Aged Care Accreditation Standards, quality systems and continuous improvement models
- Demonstrated ability to identify risk and put strategies and actionsin place to prevent, control, minimise or eliminate identified risks
- Demonstrated ability to manage and work within a set budget
- Excellent communication skills
- High level of computer literacy

Employing more than 200 people, MHS operates a 12 bed acute hospital, 2 Aged Care Residences with a total of 82 beds, primary and community care services, social support group programs and Allied Health Services.
