Post Acute Care Coordinator


  • Maternity Leave Fixed Term 12/11/18 – 10/11/19  (0.4EFT Tuesday & Wednesday)
  • A fantastic opportunity to join our innovative IPAC Team
  • Coordinate short term individually tailored packages of health and community services to people who need additional support for recuperation following discharge from a Victorian Public Hospital

Applications are sought from suitably qualified Nurse or Allied Health with previous care coordination experience to work in the IPAC Unit at Echuca Regional Health. 

Key responsibilities include:

  • Provision of care coordination: negotiating, purchasing and monitoring the provision of community based supports by contracted providers.
  • Flexibility
  • Rapid response to manage multiple referrals internal and external within short time frames
  • IT skills to capture reportable data requirements 

Our team requires someone who has:

  • Qualifications in Nursing or Allied Health
  • Proven experience as a care coordinator/ case manager
  • High-level awareness of  Heath Independent Guidelines (HIP) and referral pathways
  • Demonstrated knowledge and passion to help make a difference in health care. 

This position requires completion of a satisfactory National Police Check prior to appointment.
