Memory Support Nurse (Dementia Support)
Ongoing Part Time role (48 hours per fortnight)

About the role

The Memory Support Nurse provides specialist care, clinical expertise and leadership in the area of dementia, working with clients, carers, families and health professionals to guide and coordinate person centred assessment and care planning and delivering quality best practice care. You will be required to be a Registered or Medication Endorsed Enrolled Nurse with experience and expertise in Dementia management. Please see attached Position Description for further details.

KEY SELECTION CRITERIA

Essential:

  • Experience in working with people with dementia/memory loss and a passion to improve people's quality of life through the education and support of all those impacted by dementia/memory loss.
  • Demonstrated ability to support a positive organisational culture and work in a manner that promotes the service within the community.
  • Demonstrated time management, organisational skills and ability to work as an autonomous practitioner.
  • Ability to resource information and provide education and ongoing support to clients, families, carers and colleagues to optimise quality of life.
  • High level home based assessment skills that support relevant referrals  (and coordination of the same)  to optimise quality of life for clients and their families
  • Commitment to providing effective solutions that respect diverse client needs
  • In-depth understanding of risk identification and risk management processes within the community environment and experience with risk reporting including those relating to OH&S, Infection Control, environmental risks, mandatory reporting
  • Good communication skills as a team member, as there is crossover between various programs
  • Motivated and positive demeanour & Is able to work as a sole practitioner
  • Knowledge of current aged care standards, MyAged Care, NDIS would be beneficial
  • Passion for supporting those in the community with memory loss
  • Support for and ability to refer both clients and carers.
  • Drivers licence

Desired:

  • Post graduate qualifications or ongoing studies in Dementia or a related field
  • Experience with the My Aged Care portal and referral system
