Ongoing Part Time (32 hours per fortnight)

About The Role:

The purpose of this role is to provide client-centred assessment and planning to identify barriers to self-management and independent living, making referrals to services to address these issues and coordinating the client's care as part of a multidisciplinary care coordination team. This role also involves the facilitation of timely and appropriate referrals of SRH patients to Hospital Admissions Risk Program (HARP) services through the first point of contact and advice to SRH clinical staff.


The care coordinator assesses clients with multiple co-morbidities and associated psychosocial issues and determines those factors most likely to lead to hospital re-admission. They then work with the client and in collaboration with HARP specialist clinicians to develop strategies to diminish the impact of those factors. Services are delivered in clients' homes, community health services and outpatient settings. HARP clients include people with diabetes, chronic respiratory and chronic cardiac conditions, complex co-morbidities associated with advanced age and a range of complex psycho-social issues.


The Care Coordinator sits within the Care Coordination Team within, which aims to improve clients' health outcomes and reduce the risk of hospital admission.
