Care Coordinator | Community Programs

Hospital Admission Risk Program (HARP) and Post-Acute Care (PAC)

 

Part time 0.4 EFT - Fixed term to 28/5/23

About the Role:

The vacancy is for 16 hours per week and offers the opportunity to learn and work in community programs.

The Care coordinator works with consumers to promote health independence and positive approaches to recovery from illness and managing chronic health conditions.

The role provides integrated and coordinated person centred care to improve client health outcomes and reduce hospital length of stay and avoidable admissions.

About You:

The preferred applicant would have a strong interest in working with people in the community and with clients that have complex health care needs and chronic conditions.

The role would suit a self-motivated, team focused individual with a commitment to providing high quality holistic care to clients in the HARP program.

Knowledge of chronic disease management and demonstrated experience of effective communication skills with healthcare providers will be valued.
