Role description

As a Social Support Worker, you will play a vital role in facilitating our Social Support programs, providing essential services to frail, aged, or permanently disabled individuals and their carers. You will collaborate with a passionate team to design and implement diverse activities aimed at promoting wellness, reablement, and community participation. From gentle physical activity and exercise groups, engaging centre-based activities, opportunities for celebration, to social interaction and fun and varied community outings, your creativity and dedication will help empower our participants to live fulfilling lives.
Key Responsibilities:
- Plan and facilitate diverse activity programs tailored to meet the needs and interests of our participants.
- Provide advocacy, support, and information to empower individuals and their families to access relevant services and resources.
- Collaborate with team members and external stakeholders to ensure holistic and coordinated care for our participants.
- Maintain accurate records and documentation in accordance with organizational policies and funding requirements.
- Uphold Dhelkaya Health's values of integrity, compassion, and respect in all aspects of your work

About you
What we're looking for:
- Experience undertaking group activity programs, especially with older adults and individuals with disabilities.
- Strong consultation, planning, and facilitation skills to create meaningful and engaging activity programs.
- Excellent communication skills, both verbal and written, and the ability to work collaboratively within a team
- A commitment to delivering person-centered care that promotes independence, connection, and meaningful engagement then we encourage you to apply for this fulfilling role in a supportive and positive team!
This position requires a Certificate III in Aged Care or Disability Services, demonstrated experience in supporting aged and frail or disabled individuals, ability to work both independently and collaboratively within a team, and proficiency in providing recreational and support services in a group setting. A valid driver's license is also an essential requirement for this role.
