Position Details
THE ROLE
MACNI Coordinator
The Complex Needs Coordinator provides a single point of contact for service providers, clinicians or clients seeking general information about complex needs service responses/interventions. Complex needs service responses/interventions include Multiple and Complex Needs Initiative, the Support for High Risk Tenancy program and broader complex needs responses.
The position will:
- Provide consultation and coordination services and facilitate service planning for clients with complex support needs, including MACNI and SfHRT clients
- Facilitate relationships between departmental program areas and external services, as well as provide capacity building regarding support provision for clients with complex needs
- Partner with service providers to develop integrated responses and promote practice excellence
- Identify systems gaps, risk factors and training opportunities that will improve service systems for clients with complex needs
- Support the Client Support and Housing unit, liaise with the central Complex Support and Systemic Improvement unit, manage the program budget and represent the program in various environments.
ACCOUNTABILITIES INCLUDE
- Manage the referral processes to MACNI, provide high level advice to stakeholders in relation to complex clients through enhanced and sophisticated consultations, and support the development of key coordination and planning processes.
- Facilitate problem solving and planning discussions to help provide clarity on complex client presentations and to identify and respond to systemic issues that are blocking or impacting the ability to address complex client issues.
- Engage with a number of service providers to develop partnering relationships, identify more effective and strategic responses, enhance capacity for service integration and coordination, and maximise collaborative responses.
