About the role
South West Healthcare (SWH) is seeking a passionate and dedicated professional to join our Pathways to Good Health (PTGH) program, a state-wide initiative aimed at improving health outcomes for children and young people entering Out-of-Home Care in Victoria.
This role will contribute to ensuring that children and young people entering care, whether for the first time or subsequently, have their health needs identified, reviewed, and supported through access to timely and appropriate health services.
Key responsibilities include:
- Coordinating and facilitating age-appropriate health checks
- Identifying and referring children for required screening, assessment, and treatment
- Liaising closely with Child Protection, carers, and other relevant stakeholders
- Supporting children with complex needs through multidisciplinary assessments and the development of individualised health management plans
- Providing outreach services in community settings such as schools, early learning centres, and residential care facilities
The PTGH team at SWH is a collaborative and experienced group of professionals, including medical paediatricians, paediatric nurses, allied health clinicians, and care coordinators. The program is currently funded until June 2027, and services are delivered in an accessible and flexible manner to meet the diverse needs of children in care. This is a unique opportunity to be part of a meaningful program that directly contributes to improving the lives and long-term wellbeing of some of the most vulnerable children in our community.
Position overview
- Employment Type: Part time (48 hours per fortnight), Fixed Term until June 2027
- Location: Warrnambool Campus
- Classification: Community Health Nurse (In-Charge)
- Salary Range: $122,907.20 pro rata, plus superannuation
What you bring
You are a Registered Nurse with at least 5 years of clinical experience and a strong commitment to improving health outcomes for children and families, particularly those experiencing vulnerability or involved with the Child Protection system. You bring:
- Current AHPRA registration, Victorian Driver's Licence, and relevant police and Working with Children checks (or willingness to obtain)
- A deep understanding of trauma, child development, attachment, and the factors impacting vulnerable families
- Experience providing care to children at risk, with strong knowledge of referral pathways and service systems
- Excellent clinical assessment, communication, and report writing skills, including the ability to tailor information for non-health audiences
- Strong organisational and time management skills, with the ability to meet timelines and manage competing priorities
- A collaborative, team-focused approach and proven ability to work effectively within multidisciplinary teams and with external stakeholders
- A passion for improving health literacy and wellbeing in children and families through outreach and education
- (Desirable) Experience with out-of-home care, TrakCare, CAReHR, and a sound understanding of the broader health and wellbeing service systems
