Salary per hour: $59.08 + superannuation + generous salary packaging options

Employment Type: Ongoing full-time OR Ongoing part-time

Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028

 

Role description

Dhelkaya Health is seeking an experienced and passionate Clinical Coordinator to lead our District Nursing team. This pivotal leadership role ensures the delivery of high-quality, patient-centred care across our 7-day-a-week, extended-hours District Nursing and Community Palliative Care services.

As Clinical Coordinator, you will:

• Provide operational and clinical leadership to the District Nursing team.
• Oversee triage and intake of referrals, initial assessments, risk management, and care planning.
• Support nurses in delivering expert care, health education, and advocacy for clients.
• Coordinate workload flow, discharge planning, and targeted clinical interventions.
• Collaborate with key stakeholders including Community Palliative Care Program Lead, At Home Coordinators, and rehabilitation programs.
• Drive quality improvement initiatives in partnership with the Community Nurse Unit Manager.

 

About you

• Registered Nurse with current AHPRA registration.
• Strong leadership and team coordination skills.
• Experience in community nursing and/or palliative care.
• Excellent communication and stakeholder engagement abilities.
• Commitment to best practice and continuous improvement.
