Position Title: Registered Nurse Care Coordinator – Complex Care (HARP)        

Classification: ZF4             

Salary per hour:  $59.72 + superannuation + generous salary packaging options

Employment Type: Ongoing Part-time 48 hours per fortnight

Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028

 

Role description

As a Registered Nurse Care Coordinator within the HIP Complex Care Service (CCS), you will provide person-centred care coordination for clients with complex psycho-social and healthcare needs. This role focuses on supporting clients to improve their self-management skills, health, and wellbeing through a collaborative approach.

You will:

• Conduct comprehensive holistic health assessments in clients' homes and local communities.
• Coordinate all aspects of health and social care needs, including referrals to specialist services.
• Monitor progress toward client goals and facilitate interventions from other health professionals.
• Ensure seamless linkages between hospital and community services.
• Deliver high-quality, evidence-based clinical interventions in line with professional standards and scope of practice.

 

About you

• Registered Nurse with current AHPRA registration.
• Experience in chronic disease management, complex aged care, or community health.
• Strong assessment, care planning, and coordination skills.
• Ability to work autonomously and collaboratively within a multidisciplinary team.
• Excellent communication and problem-solving skills.
