About this role
The Transition Care Program (TCP) supports older people to regain confidence, independence and functional ability after a hospital stay. TCP is goal‑focused, time‑limited and therapy‑driven, offering tailored supports such as physiotherapy, occupational therapy, social work, nursing intervention and personal care to help clients maintain and improve their physical and cognitive wellbeing.
At Central Highlands Rural Health, Transition Care is delivered on behalf of Grampians Health and the Victorian Department of Health.
We offer both bed‑based care at Creswick Hospital, Daylesford Hospital and Trentham Hostel, as well as community‑based TCP within clients' homes across the Hepburn Shire.
We are now seeking a passionate and experienced Registered Nurse to join our Care at Home team as our Transitional Care Program Coordinator.

The successful candidate will bring:
- Strong clinical assessment, organisational and critical‑thinking skills
- A commitment to delivering safe, high‑quality, person‑centred care
- Experience in one or more of the following:
- Transition Care Programs
- Leadership or management roles
- Complex & chronic disease management
- Hospital in the Home or other specialist community‑based programs
