Role description
Dhelkaya Health is seeking an experienced and motivated Diabetes Nurse Educator to join our multidisciplinary team in a Clinical Nurse Consultant capacity. This role provides specialist diabetes education, clinical expertise, and support to individuals living with diabetes, enabling improved self‑management, health literacy, and long‑term health outcomes.
The Diabetes Nurse Educator works within a clinical advisory role, providing advanced nursing care in line with the Registered Nurse Standards for Practice, relevant legislation, and professional codes. The role involves comprehensive assessment, care planning, implementation, and evaluation for clients and their significant others, delivered in a person‑centred and evidence‑informed manner.
You will practice both independently and collaboratively, demonstrating clinical leadership, accountability, and responsibility for your own practice and the appropriate delegation of care to enrolled nurses and other healthcare workers.
Key Responsibilities
- Provide specialist diabetes education and management support across the continuum of care
- Support and empower clients to improve diabetes self‑management and health literacy
- Undertake comprehensive clinical assessments and develop individualised, client‑centred care plans
- Work collaboratively with multidisciplinary teams to achieve optimal health outcomes
- Provide clinical leadership, guidance, and support to nursing and healthcare staff
- Delegate care appropriately, considering scope of practice, education, and context of care
- Practice in accordance with evidence‑based guidelines, professional standards, and organisational policies
About you
- Registered Nurse (Division 1) qualification and current AHPRA registration
- Graduate Certificate of Diabetes Education (or equivalent)
- Current credentials with the Australian Diabetes Educators Association (ADEA)
