About the role
The Early Intervention in Chronic Disease Coordinator plays a key leadership role in promoting early intervention, prevention, and self‑management for individuals living with, or at risk of, chronic disease. The role leads the planning, coordination, and delivery of community awareness and prevention activities that support early identification of chronic conditions and empower people to take an active role in managing their health.
A core focus of the position is building workforce capability through the delivery of education, training, and professional support for staff involved in the care of people with newly diagnosed chronic illness. The coordinator works collaboratively with multidisciplinary teams, community groups, and key stakeholders to ensure evidence‑based, person‑centred early intervention approaches are embedded across services.
Through strengthening health literacy, supporting behaviour change, and promoting integrated preventative care, the Early Intervention in Chronic Disease Coordinator contributes to improved health outcomes and reduced long‑term impact of chronic disease on individuals and the community.
What you bring
- Registration as a Registered Nurse with a current Registration Certificate with AHPRA, supported by relevant post‑basic and/or tertiary qualifications and a minimum of three years' post‑registration experience.
- A strong preventative and educative approach to healthcare, with a demonstrated commitment to health promotion, early intervention, and supporting individuals to self‑manage chronic conditions.
- A solid understanding of the role of the nurse as an educator and partner in care, working collaboratively with clients, carers, communities, and multidisciplinary health teams to achieve improved health outcomes and prevent disease progression.
- Proven ability to work independently and collaboratively with a diverse range of clients, carers, and service providers, building positive relationships across community and health service settings.
- Demonstrated capability to assess community needs, and to plan, coordinate, deliver, and evaluate high‑quality health promotion and early intervention programs that respond to identified priorities.
- Highly developed communication skills (written and verbal), with strong interpersonal, problem‑solving, negotiation, and organisational abilities to support effective service delivery and stakeholder engagement.
