Salary per hour: $31.65 + superannuation + generous salary packaging options
Employment Type: Ongoing part-time 56 hours per fortnight

Role description

As a Social Support Worker, you will deliver meaningful, person-centred support to older people and individuals with disability, helping them stay connected, independent and engaged in their community. Through facilitating group activities, supporting community access, and contributing to goal-directed care planning, you will play a key role in improving wellbeing and reducing social isolation for some of the most vulnerable members of our community.

Key Responsibilities
- Plan and facilitate diverse activity programs tailored to meet the needs and interests of our participants.
- Provide advocacy, support, and information to empower individuals and their families to access relevant services and resources.
- Collaborate with team members and external stakeholders to ensure holistic and coordinated care for our participants.
- Maintain accurate records and documentation in accordance with organizational policies and funding requirements.
- Uphold Dhelkaya Health's values of integrity, compassion, and respect in all aspects of your work

About you
What We're Looking For:
- Experience undertaking group activity programs, especially with older adults and individuals with disabilities.
- Strong skills in conducting intake and assessments and collaborating with clients/participants/carers on goal directed care planning.
- Excellent communication skills, both verbal and written, and the ability to work collaboratively within a team
- Understanding of and experience utilising the My Aged Care portal
