Moyne Health Services has an exciting opportunity for a Physiotherapist to work within a dynamic multi-disciplinary Community Health Team. This position is located at our Port Fairy and Koroit campuses with some off site work within the community setting. Port Fairy is a vibrant coastal town with spectacular beaches, cafes and restaurants. The community are passionate about keeping fit and healthy and are very proud of the Moyne Health Organisation with a new purpose built Community Health Service, Acute ward and refurbished Residential care facility. The position is a flexible appointment, inviting applicants seeking either a part-time (0.6-0.8) or full-time position.

Position Details

About the role

The Physiotherapy Team works across Community Health, Residential Care and the Acute Ward. This provides a challenging and diverse case load.

In Community Health:

  • Providing comprehensive assessments and treatments for eligible clients (CHSP, STRC, HACC, NDIS, DVA)
  • Over-seeing and delivery of group programs
  • Promotion of self- management strategies through health coaching

In Residential Care:

  • Providing comprehensive resident assessment and reviews
  • Reviewing falls and implementation of prevention strategies
  • Provision of ACFI pain management program
  • Delivery of rehabilitation programs
  • Planning and implementation of group exercise 

In Acute Ward:

  • Providing comprehensive patient assessment and treatment
  • Review of falls and implementation of prevention strategies
  • Comprehensive discharge planning and implementation

Across all sites the Physiotherapist will be involved in consistent, constructive and collaborative communication with patients and their families, staff and referrers, to ensure best care at all times for all patients

About you

To be successful in this role, you will have the following qualifications and skills:

  • Current registration as a physiotherapist with AHPRA
  • Ability to effectively plan, implement and evaluate physiotherapy treatment programs
  • Commitment to quality, best practice and environmental safety
  • Hold a current Victorian Drivers Licence
  • Effective communication and interpersonal skills
  • Demonstrated ability to efficiently manage case load demands, problem solve and prioritise
  • Proven ability to work effectively in a team environment and independently as required
  • Demonstrated ability to provide effective customer focussed services
  • Commitment to professional development and maintenance of current evidence based practice
  • Knowledge of quality/continuous improvement process
  • Flexible positive approach to work
