Position Details

About the Role

As a Health Care Worker in our Community Health team, your primary objective is to provide a high level of care and assistance to community clients.
Assistance may involve personal care, client transport to appointments, outings or shopping and cleaning duties within client homes.
During these interactions it is recognised that the health care worker will be assisting clients of varying abilities, requiring varying levels of support during their daily activities.

About You

Minimum Qualifications:

• Certificate III or IV in Home and Community Care or equivalent


Key Selection Criteria

• Knowledge of the wellness and re-ablement approach to aged care with regard to goal setting and care planning.
• Knowledge of issues relating to frail / aged, people with disabilities and their carers
• Understanding of Accreditation Standards, quality systems and continuous improvement models
• Demonstrated written and verbal communication skills
• Computer literacy

Moyne Health Services is a public, not for profit, integrated healthcare service that helps meet community healthcare needs in Port Fairy and surrounding districts in the Moyne Shire. Employing more than 200 people, MHS operates a 12 bed acute hospital, 2 Aged Care Residences with a total of 82 beds, primary and community care services, social support group programs and Allied Health Services.
